開眼頭手術除了能夠放大眼睛外，也兼具改變眼型或雙眼皮樣式的功效。通常眼頭較厚的鳳眼患者於割完雙眼皮後，其眼內角1/5處的褶線會自然地埋入眼頭皮膚內並不會外露，這是所謂的封閉式雙眼皮e（或是鳳眼型雙眼皮），屬於東方人固有的雙眼皮型態，其效果雖然自然，但對於眼距或眼睛大小並無所改變。然而如果將眼頭切開往眼內眥處縫合的話，那雙眼皮的摺線就可以一路延伸至眼內角的深處，不會再被眼頭的皮膚所覆蓋，如此即可塑造出類似西方人或混血般的開放式雙眼皮（或稱平行線雙眼皮），甚至還能同時調整眼頭的走向或水平，量身訂做一對適合自己的眼睛，因此甚受許多崇尚明眸大眼女性的喜愛。

開眼頭的手術做法有許多種選擇，目前莊醫師主要採用的是日式的W型眼頭成形術，此法必須先切除部分多餘的眼頭皮膚，接著將眼內眥橫向切開後往上下兩端的皮膚缺口處拉緊縫合，最後再將新的傷口順修並與雙眼皮褶線對齊銜接，是屬於比較精細的微創開眼頭手術方法. 它的優點是眼頭內切的角度與幅度可隨意調整，不會發生如傳統剪眼頭手術般容易導致的過度內吃或紅肉暴露等問題，且其切口可以與雙眼皮褶線完整的契合，避免眼內角褶痕不順等後遺症。另外就是此法的手術效果自然，可避免掉貓眼、狐狸眼或鬥雞眼等問題，不僅安全性高且能符合絕大多數患者的眼型需求。至於它的缺點則是縫線眾多且需精細對齊皮膚，手術的步驟較為複雜，另外就是在眼內角下端皮膚會露出一段縫合的傷口，可能會有短暫性的疤痕問題，通常需要經過3~6個月的復原才能逐漸消退直至肉眼看不到的程度。
