內縮法
此法是在乳暈與乳頭交界處切除部份鬆弛的乳暈皮膚後再做向心狀縫合, 其優點是手術簡單且快速, 而缺點是容易於乳頭根部周圍留下一圈白色的疤痕(如甜甜圈狀), 容易與周邊深色的乳暈產生明顯色差與對比, 此外乳頭也可能因傷口縫合過緊而導致變形或甚至平坦, 再加上乳暈縮減程度有其限等缺點,莊醫師多不建議採用此法於乳暈明顯擴大的患者.

外縮法
此手術則是從乳暈外緣與正常皮膚交界處進行部分乳暈的切除, 再做向心圓狀縫合. 此法初期會造成傷口周邊皮膚的皺褶, 但約1~2個月後即會自動平整, 其最大的優點是乳暈的切除縮減範圍較內開法來得多, 不僅能將疤痕融合在乳暈與正常皮膚的交界處, 適合東方人的皮膚特性, 更可同時合併做乳暈切口的提乳手術, 手術的運用上較具彈性. 不過此法仍然可能會於乳暈周圍形成一圈淡色的疤痕, 因此術後3~6個月內須注意做好疤痕的照護.
